Private Letters and Forms

PRIVATE LETTERS AND FORMS- Updated From the 16th June 2026

Requests for private letters, reports and completion of forms are not part of NHS services and are therefore considered private work.

NHS work always takes priority. Private letters, reports and forms are completed at the discretion of the GP and only when clinical workload allows.

Before any private work is undertaken, patients will be required to:

  • Complete and sign a Private Letter / Report / Form Request Agreement.
  • Acknowledge and accept the applicable fee.
  • Make payment in full in advance.

Please allow up to 28 working days from receipt of payment and all required documentation for completion of private work. Urgent requests cannot usually be accommodated.

The GP may decline any request if:

  • There is insufficient information available to provide the requested opinion.
  • The request is considered inappropriate.
  • The request falls outside the GP’s expertise.
  • NHS workload does not allow the work to be completed.

Please note:

  • Fees are non-refundable once work has commenced.
  • Completion of a letter or form does not guarantee that the requesting organisation will accept the information provided.
  • Any amendments requested after a letter or form has been completed may incur an additional fee.
  • Where a request is made on behalf of, or for submission to, a third party (for example an insurance company, employer or solicitor), the patient remains responsible for payment of the applicable fee.
  • If a patient withdraws a request after work has commenced, or if a third party subsequently declines to pay, the patient will remain liable for the full fee.

We do not provide or sign:

  • Passport applications
  • Proof of identity documents
  • Character references
  • Employment references
  • Overseas forms requiring certification
  • Firearm certificates

Housing-related letters

Requests for letters relating to damp, mould or housing conditions will be considered on an individual basis and at the discretion of the GP.

Other private letters, reports and forms not listed above may be completed at the discretion of the GP.

Fees will be determined according to the complexity of the request and the time required to complete it. The practice will advise the applicable fee before work is undertaken.

All fees must be accepted and paid in full before work commences.

Please note that fees are non-refundable once work has commenced, regardless of the outcome of the request. This includes situations where a patient withdraws their request or where a third party subsequently declines to pay for work already undertaken.
